Resolved Contact Forms doesn't work

unvortunately the contact form doesn't work:

Please check screenshots, the global configs show that the PHP Mailer is working well... and the settings of the forms are filled...

1. Create a new page and load the contact addon. Then check again
2. Which template did you use?
3. Did you try google mail or your server mail?
4. Your site is off-line. Please make it live to check error.
